Medical imaging has transformed the way kidney cancer is detected. Today, many kidney tumours are discovered incidentally during ultrasound, CT scans or MRI scans performed for unrelated concerns such as abdominal pain, digestive problems, back pain or routine health check-ups. These are known as incidental kidney tumours or incidentalomas.

Unlike many cancers, early-stage kidney cancer often causes no symptoms. As a result, many patients are unaware they have the disease until it is detected during imaging for another medical condition. The advantage of incidental detection is that these tumours are often diagnosed at an early stage, when treatment is more successful and the chances of preserving kidney function are higher.

What is an incidental kidney tumour?

An incidental kidney tumour is a mass found unexpectedly during imaging that was not performed to investigate kidney disease. These masses may be benign (non-cancerous) or malignant (cancerous).

Common benign kidney masses include:

• Simple kidney cysts
• Oncocytomas
• Angiomyolipomas

Although imaging provides important clues, further evaluation is often required to determine whether the tumour is cancerous.

Why are more kidney tumours being detected today?

The increasing use of advanced imaging techniques has led to a rise in early diagnosis. These include:

• Ultrasound
• CT scan
• MRI
• Preventive health screening packages

Many kidney tumours are now detected while investigating unrelated health problems, allowing treatment before the disease progresses.

Who is at higher risk?

Certain individuals have a greater risk of developing kidney cancer, including those with:

• Smoking history
• Obesity
• High blood pressure
• Chronic kidney disease
• Family history of kidney cancer
• Inherited genetic syndromes
• Long-term dialysis

People with these risk factors should discuss appropriate monitoring with their healthcare provider.

What happens after a kidney tumour is found?

Once a kidney mass is identified, doctors assess:

• Tumour size
• Location within the kidney
• Imaging characteristics
• Growth pattern
• Kidney function
• Overall health of the patient

Additional investigations may include contrast-enhanced CT or MRI scans, blood tests and, in selected cases, a biopsy.

Does every kidney tumour need surgery?

Not always. Treatment depends on the tumour and the patient’s overall health. Management options include:

• Active surveillance for selected small tumours
• Partial nephrectomy (kidney-sparing surgery)
• Radical nephrectomy (complete kidney removal)
• Minimally invasive ablative treatments such as cryoablation or radiofrequency ablation

Why is early detection important?

Kidney cancers detected early generally:

• Have a higher cure rate
• Require less extensive treatment
• Allow better preservation of kidney function
• Carry a lower risk of recurrence

By contrast, cancers diagnosed after symptoms develop are often larger and may have already spread, making treatment more challenging.

Can kidney cancer be prevented?

Although kidney cancer cannot always be prevented, the risk can be reduced by:

• Avoiding tobacco
• Maintaining a healthy weight
• Controlling blood pressure and diabetes
• Exercising regularly
• Staying well hydrated
• Undergoing regular health check-ups, especially if you have risk factors

Routine screening is not recommended for the general population. However, individuals at higher risk may benefit from personalised surveillance. Early detection, timely evaluation and appropriate treatment remain the key factors in achieving the best possible outcomes for patients with kidney cancer.
